                     2. Complete the form. Click here for the location of the tag number.

                     3. Mail the completed form along with a $5 check payable to Gaines Bend
                         Property Owners Association to the address on the bottom of the form.

3. Once your toll tag application is complete you can either place it in the GBPOA Mail Box located in the mail center, drop it by the Sales Office if open or email the form to gainesbendpoa.com. Your PMI account will be billed for a $5.00 service fee.
